——- SURGERY NEWS——

For the benefit of our patients, especially those with poor mobility, the surgery will be installing a patient lift. This has also given us the opportunity to make other improvements to the surgery by changing the downstairs waiting room, the reception area and the first floor.  

Unfortunately due to the amount of work involved this will cause some disruption. However as much of the work will be done at weekends, we hope to keep the distruption to a minimum.

We look forward to the work being completed and the lift operational by the Autumn 2015.

 

NHS ENGLAND LETTER

Please be aware you  may receive a letter from NHS England asking you to respond to them to let them know you are still living at your current address.

Please respond as if you do not you may be deducted from our patient register and have to re-register with us.

TO  ORDER YOUR REPEAT PRESCRIPTION ON LINE 

 You can request your repeat prescriptions 'online' through our clinical system called 'Systmone'

To use this way of ordering your prescriptions you will need a Username and Password. Please come to Reception with valid ID and proof of address to complete an application form.

Once you have these please follow the instructions below.

Instructions (once you have your username and password)

· You will still log in through the Surgery website www.3swanssurgery.nhs.uk  click on ‘order your  repeat prescription Online’.

· You will then be asked for your Username and Password details.

· On the home menu please select Current Prescriptions and follow the instructions.

Once logged in you will also see that you can book/cancel doctor and/or nurse appointments up to 5 weeks in advance.

Should you own an Apple iphone or ipad then you will find that a SystmOnline app is available to download from the iTunes app store (SystmOnline is the name of the app). This will give you all the benefits above without having to log in via the website.

 

 

Repeat Prescriptions  

If you are taking regular medication please place your order before 11.00am and allow at least 48 hours notice (excluding Bank Holidays and weekends) to ensure your prescription is ready for collection.

If you would like your prescription sent to a chemist, please allow an extra 24 hours to enable the chemist to process your prescription

URGENT PRESCRIPTIONS REQUIRED THE SAME DAY MUST BE RECEIVED BEFORE 11.00AM AND CLEARLY MARKED URGENT. THEY WILL BE READY FOR COLLECTION AFTER 3.00PM

WE REGRET WE CANNOT ACCEPT PRESCRIPTION REQUESTS OVER THE TELEPHONE

Please use the e-mail form (click onto the link at the top of this page) to order your items on line or tick the items you require on your repeat medication form and place it in the box in reception. Alternatively, you can post in your request or fax it to us on 01722 503626

When checked and signed by the doctor, your prescription will be ready to collect from either reception or if you have enclosed a stamped addressed envelope, we will post back your prescription to you.  Alternatively, we now have The Three Swans Pharmacy on the premises and most of the other chemists collect from the surgery every day. Please advise us by writing the chemist of your choice on your repeat slip/e-mail form every time you request a repeat prescription, if you would like it to go to a chemist.
